Presbyterian Hospital Committees, 1994

PHARMACY COMMITTEE


Early reference show Committee was concerned with use of different drugs used in Hospital and preparation of Pharmacopoeia. Approved by Executive Committee of Medical Board as a Standing Committee on October 4, 1949.

STATEMENT OF CHARGE TO COMMITTEE:

1. Maintenance of the Hospital Formulary:

a. New pharmaceutical agents reviewed and added to the formulary as needed;

b. Elimination from formulary of drugs whose use is limited;

c. Designation of certain compounds for use only by individuals belonging to selected groups.

2. Use of compounds in the Hospital is periodically monitored to present to departments suggestions concerning "use-practices" of their staff.

3. Information of importance or interest to staff about pharmaceutical products disseminated by memos or letters.

4. Voting members of this committee shall include: The Director of Pharmacy, Epidemiology, representatives from Nursing, physicians from various clinical services and others as deemed appropriate by the Chairperson.

5. Reviews and monitors all significant untoward drug reaction reports.

6. Meets monthly.

7. Reports quarterly to Executive Committee of Medical Board.

8. Reports annually to Medical Board.
