Appointments
Patients are seen by appointment only which can be reserved by calling 212.305.5293.
Cosmetic dermatology patients can reserve appointments by calling 212.326.8889.

At the time of your appointment, please be sure to notify our staff of any demographic updates such as new address, phone number, or email address.

Cancellation Policy
Should you need to cancel your appointment please provide at least 24 hours notice.
Effective September 1, 2006 failure to do so will result in a fee of $100 that must be paid prior to your next appointment. This fee cannot be submitted to Medicare or your insurance company.

Payment and Insurance
Payment is due at the time of service. For your convenience, we accept cash, check, debit card, MasterCard, Visa, American Express and Discover. Additionally, we also offer CareCredit Healthcare Financing for elective procedures.

Columbia Dermatology Associates participate in numerous insurance programs including Medicare. To confirm the participation of your dermatologist with your insurance plan please contact our offices at 212.305.5293.

Please be certain to provide reception with all of the required referral and authorization forms prior to or at the time of your visit. Without the necessary referral forms, full payment for services rendered is due at the time of your appointment or a cancellation of your appointment will ensue.
