Text, references, legends and tables should be printed on standard 8.5" X 11" paper, double-spaced. Pagination should begin with the title page and include, in the following order: title page, abstract, text, references, acknowledgment of support, tables, figures and legends.

Title Page

Include in the title page of the original manuscript the authors' names with highest academic degrees earned (Columbia students and graduates, provide class year), date submitted, and address and telephone number of corresponding author. Also include a running title of three to six words, and three to six keywords for indexing purposes. Omit names and other identifying information on the title page of the three copies.

Abstract

The abstract should be limited to 250 words and should briefly outline the scope and focus of the paper and summarize the results and conclusions. Avoid the use of abbreviations.

Text

Include subheadings in boldface when appropriate. Measurements should be reported in S.I. units. Every reference, table and figure should be cited in numerical order. Tables, figures and their legends should be cited by Arabic numerals and appended following the manuscript pages. Abbreviations should be kept to a minimum with each abbreviation defined when first used. Brief footnotes should be used only on the title page and tables, not within the text. The recommended maximum manuscript length (abstract plus text only) is 3,000 words. Provide a word count in the cover letter.

References

References should be numbered in order of mention and cited by superscript numerals in the text. Provide concluding page numbers for all journal references and specific page numbers for all book references. Indicate all abstracts and letters. Reference format should follow the general arrangement, abbreviations and punctuation given in: Uniform requirements for manuscripts submitted to biomedical journals. N Engl J Med 1991;324:424-428. For standard abbreviations of periodicals, follow INDEX MEDICUS. List all authors, except when they number six or more in which case list only the first three, followed by et al.
